Patient Report Essay -- Medical Report
A treadmill stress examination for atypical chest annoyance demonstrated inferior wall ischemia. Cardiac catheter demonstrated patent graphs and left wing ventricular ejection fraction of 68%. Treadmill stress test felt to be false positive however, long-suffering of of did have 5 beats of ventricular tachycardia. Holter demonstrated 3 beats of multifocal ventricular tachycardia. The patient was placed on Cardizem and atypical chest pain resolved. The patient complained of periods of difficulty breathing, especially when he awakened at iniquity to urinate. Cardizem was increased and pulmonary function tests were normal with no shew of respiratory failure. The patient also reported flu 3 weeks antecedent to ___ DATE. Patient also seen about 2 weeks ago by ____ NAME and is on Coumadin for episode of atrial fibrillation. Holter to determine if Coumadin could be stopped. The patient came to clinic on day of admission to be seen, noting severe shortness of breath, pres ently almost constant as if he could not get affluent air. The patient had to deep breathe.
